The Bloodied Wolf first appears in the first cutscene of Diablo 4 Items, after your Drifter escapes to a cavern in the wake of losing their pony. During the evening, a wolf-like figure appears before the cavern, yet the Drifter rapidly blacks out. At the point when they wake, the wolf is no more.
Yet again later, in Act 1, when the Drifter and Neyrelle are searching for Lilith in the Vault of the Horadrim, this wolf appears to the Vagabond.
